NEW PATIENTS
All new patients are required to attend the Patient orientation Class before being seen at the clinic. To quality for treatment at the clinic, patients must have NO health insurance (this includes, but is not limited to: Medicaid, Medicare, private insurance, commercial insurance, employer health insurance, union health insurance, VA health benefits, etc), and income cannot exceed 200% of the Federal poverty level.

New Patient Qualification Forms


New patient orientation is held the 2nd and 4th Thursday of each month (except those falling on a holiday) beginning at 9:00 AM and it will last about an hour. Patients will be provided general information about the clinic, clinic guidelines, and patient will also fill out all forms for qualification and meet with a GDH representative for qualification. Patients requesting to see a provider on a Thursday that is orientation day can arrive at the clinic by 9:00 AM with documents required,and that patient will be guaranteed to be seen that afternoon by a provider. If a patient requests to see provider on a Thursday that is not orientation day, then that patient must wait until following week.

There is no need to call beforehand, just arrive for the meeting at 9:00 AM.

Patients are REQUIRED to bring with them:

a. PROOF OF INCOME or lack of it.
 This can be in the form of one or all of the following:
  - Current year's income tax return (first two pages).
  - Current months check stubs.
  - Two months bank statement.
  - Labor department print out of earnings.

b. LETTER OF SUPPORT - If patient is supported by another person, you must bring a letter stating such and signed by that person

c. PHOTO ID

d. SOCIAL SECURITY CARD

c. LIST OF MONTHLY EXPENSES

e. Any type of Social Security documents they have received for the entire family. i.e. Medicaid letter, SSI award letter, food stamp award letter, support award letter for dependent children.

f. List of all medications they are currently taking and dosages. Patient may bring prescription bottles.

g. NOTE: IF YOU ARE A NEW PATIENT, YOU WILL BE REQUIRED TO SUCCESSFULLY COMPLETE THE ORIENTATION CLASS BEFORE YOU WILL BE ABLE TO RECEIVE MEDICAL SERVICES PROVIDED BY THE CLINIC.

HOW TO SEE THE DOCTOR:

To be seen by a provider Thursday afternoon, please call between 8:00 AM - 11:00 AM on that same Thursday morning. You will then be assigned an appointment time for later that day. Please arrive 30 minutes before your appointment time. The clinic waiting room opens at 1:00 PM on Thursdays, so please take this into consideration when planning your visit to the Rock Springs Clinic.

Tuesday: 8:30 AM - 3:00 PM

Thursday: 2:30 PM - 6:30 PM

Tuesday pick-up is only available if you have been notified that your medicine is ready.

In Need of Transportation?

You can schedule transportation to the clinic through a private transportation company.

The Rock Springs Clinic is currently not staffed to handle all mental and behavioral healthcare needs. For patients requiring more specialized care and treatment, the clinic has partnered with several other clinics and facilities Barnesville, Griffin, and Thomaston.
