NEW PATIENTS
All new patients are required to attend a New Patient Orientation class before being seen at the clinic. To quality for treatment at the clinic, patients must have NO health insurance (this includes, but is not limited to: Medicaid, Medicare, private insurance, commercial insurance, employer health insurance, union health insurance, VA health benefits, etc), and income cannot exceed 200% of the Federal poverty level.

Patient Guidelines

New Patient Qualification Forms


New patient orientation is held the 2nd and 4th Thursday of each month (with some exceptions, including holidays) beginning at 9:00 AM and it will last about an hour. ***Please see the clinic Facebook Page for more up-to-date information on appointments and notifications of orientation cancellations.*** Patients will be provided general information about the clinic along with clinic guidelines, and patients will also fill out all forms and meet with a GDH representative for qualification.

There is no need to call beforehand, just arrive for the meeting at 9:00 AM.

You are REQUIRED to bring:

a. PROOF OF INCOME or lack of it for ALL members of household.
 This must be in the form of one or all of the following:
  - Current year's income tax return, including 1099s, schedule C forms, etc, along with W2s.
  - Labor department print out of earnings dated within past 6 months.
  - 2+ check stubs.
  - 2+ months bank statements.

b. LETTER OF SUPPORT - If supported by another person, you must bring a letter stating such, signed by that person, along with a copy of their photo ID.

c. PHOTO ID

d. SOCIAL SECURITY CARD

c. LIST OF MONTHLY EXPENSES

e. Any type of Social Security documents received for the entire family. i.e. Medicaid letter, SSI award letter, food stamp award letter, support award letter for dependent children.

f. List of all medications currently taken and dosages. You may bring prescription bottles.

g. Medicaid Denial Letter dated from past 6 months (you can attend orientation without this, but it is necessary to receive certain medications).

***NOTE: IF YOU ARE A NEW PATIENT, YOU WILL BE REQUIRED TO SUCCESSFULLY COMPLETE THE ORIENTATION CLASS BEFORE YOU WILL BE ABLE TO RECEIVE MEDICAL SERVICES PROVIDED BY THE CLINIC.

CURRENT PATIENTS

HOW TO SEE THE DOCTOR:

To be seen by a provider Thursday afternoon, please call between 8:00 AM - 11:00 AM on that same Thursday morning. You will then be assigned an appointment time for later that day. Please arrive 30 minutes before your appointment time. The clinic waiting room opens at 1:00 PM on Thursdays, so please take this into consideration when planning your visit to the Rock Springs Clinic.




MEDICINE PICK-UP:

Tuesday: 8:30 AM-3:00 PM

Thursday: 2:30 PM-6:30 PM

Tuesday pick-up is only available if you have been notified that your medicine is ready.

h. Annual $15 donation for prescription assistance - or we will only write prescriptions to be filled elsewhere

***NOTE: IF YOU HAVE NOT SEEN THE DOCTOR IN OVER A YEAR, YOU MAY BE REQUIRED TO SUCCESSFULLY COMPLETE THE ORIENTATION CLASS AGAIN BEFORE YOU WILL BE ABLE TO RECEIVE MEDICAL SERVICES PROVIDED BY THE CLINIC.

In Need of Transportation?

You can schedule transportation to the clinic through a private transportation company.

Transition Commute Solutions - 1.855.407.7433 Cost: $2 each way. Drivers accept cash only. Patient is responsible for scheduling, payment, and any other conflicts concerning transportation.

The Rock Springs Clinic is currently not staffed to handle all mental and behavioral healthcare needs. For patients requiring more specialized care and treatment, the clinic has partnered with several other clinics and facilities Barnesville, Griffin, and Thomaston.
